Investigation Summary
On May 26, 2005, Employee #1 was part of a team working in an elevator penthouse moving disconnects, running new feeders to the panel boxes, and installing new conduit and wiring. They had finished work for the day, and had accessed the right-side maintenance trap door in the center of the room so that they could lower materials and tools to the level below the elevator penthouse. After opening the trap door, Employee #1 and a coworker began to inspect the conduit they had installed over the top of it. While looking at the pipe, Employee #1 took a step forward and fell approximately 21 ft to the concrete floor. He sustained multiple injuries, including internal injuries and severe head trauma. Employee #1 was transported to the hospital, where he died at 3:30 p.m. on June 4, 2005.
