Employee Struck and Killed by Forklift

On September 26, 2005, Employee #1 was returning from the restroom to his workstation when he entered the main aisle way into the path of an oncoming forklift. The forklift struck him, and knocked him backward onto the concrete floor. Employee #1 was killed.
