Investigation Summary
On June 22, 2009, an employee was on his knees, chalking a line on a section of blue styro foam. A load of roofing materials, Hydroflex, which was being hoisted unto a staging area of the roof, became dislodged from a wooden pallet. The employee was struck in multiple locations and was killed.
