Employee Is Crushed and Killed by Chassis Carrier

Employee #1, a maintenance supervisor, was found pinned under an automobile chassis carrier, when an electrician noticed that the carrier was tilted out of position. A conveyor system runs through this area (Drop Lifter Platform area), using these carriers to move automobile chassis. When the carrier enters this area, it drops about 15 ft to within 10-in. of the drop lifter platform. This area (Drop Lifter Platform) is accessed by a fixed ladder. Lockout is a requirement to enter this area. No one was aware that Employee #1 had entered this area. Employee #1 was crushed between the carrier and platform floor. Employee #1 was killed.
