Investigation Summary
On June 26, 2007, Employee #1 was standing on a trailer giving boarding directions to an operator driving an off road vehicle onto the trailer. Employee #1 was standing in front of the vehicle giving directions when it ran over the top of him. The off road vehicle is used to transport equipment, materials, and personnel to job sites in wet, swampy areas. Employee #1 was fatally crushed.
