Investigation Summary

Investigation Nr: 201391778
Event: 01/08/2008
Employee Suffers Burns from Arc Blast, Later Dies

On January 8, 2008, Employee #1 was checking the fuse clips on an individual bucket in a 480 distribution panel that supplied power to a lighting transformer. He was checking the clips and lugs to determine the reason that a thermal image photo showed that this area of the panel was heating up. He had opened the disconnect on the front of the individual bucket and removed the three fuses. At some point, there was an arc blast and the employee was burned. Employee #1 died of complications from his burns.
