Employee falls from plank and dies

At approximately 1:30 p.m. on July 5, 2005, Employee #1 was working as a Superintendent on a construction site for Cole Construction Co. of Sanford Inc. He had instructed a laborer to place two separate 9-ft long, 2-in, by 8-in. (ALS nominal size), wooden planks across two I-beams; perpendicular to the I-beams and resting on top of them with the end of the boards butted up against the concrete block wall. The ends of the wooden planks hung over the sides of the I-beams by 3 ft 4in. The laborer was on the second floor, and had been cutting angle brackets for the pour stops to be secured to the I-beams along the concrete wall. A welder was on an I-beam located on the north-east corner and Employee #1 instructed him to weld 2-in. by 2-in. angle clips on the I-beam. He was assisting the welder by passing the pour stops fabricated by the laborer to the welder. Employee #1 walked towards the south on one of the planks that were resting on the I-beam. The plank flipped and he fell 14 ft 5 in. unto a hard gravel ground. Employee #1 was killed. He was not wearing any fall protection equipment.
