Employee Struck and Killed by Forklift

Employee #1 was removing or placing tie down shields on the storage rack under the bed of the 35-foot flatbed trailer. He turned and was struck by the palletized load, approximately 3,400 lbs of mortar mix, being transported by an 11,500-pound-capacity forklift which was loading the trailer. Employee #1 was knocked to the ground and the forklift continued forward approximately 10 to 12 feet, with Employee #1 between the wheels of the forklift. The ground clearance at the front of the forklift was 7 to 9 inches. The forklift operator did not see the Employee #1 because the load was raised, obscuring the area immediately ahead of the forklift. The forklift operator was inside a closed, air conditioned cab and was not aware of the strike until another employee stopped him. Employee #1 was killed.
