Investigation Summary

Investigation Nr: 200354553
Event: 05/05/2005
Employee Is Killed by Falling Tree Branch

On May 5, 2005, Employee #1 climbed a tree, using his climbing spurs and arborist's saddle, to remove the upper branches and top of the tree, prior to removal of the main trunk. He was secured to the tree by an appropriate climbing line. He was on the opposite side of the tree from the expected path of the released top, twenty-five feet above the ground. After the cut was made, the top dropped slightly and twisted, swinging an attached branch into Employee #1, crushing him to death.
