Employee is kiled from falling off truck

On April 7, 2005, Employee #1 was standing on top of a dump truck bed, manually unloading construction debris from the bucket of a skid loader into the dump truck with a coworker. The dump truck was parked in the cul-de-sac of a subdivision. The employee fell off the rear of the dump truck approximately 8.5 ft unto the pavement. Employee #1 was killed.
