Employee trips and fralls from roof

On July 5, 2005, Employee #1 was part of a crew of five roofers replacing sections of damaged roof plywood decking of various sizes, none being full sheets of decking. The employee tripped on a sheet of plywood laying on the edge of the roof and fell 18 ft. Employee #1 sustained several fractured and bruised ribs and was hospitalized. Only one crew member was wearing a safety belt, the rest were not using any fall protection.
