Employee Is Killed in Trench Collapse

On March 31, 2005, Employee #1 and a coworker were making a sewer connection for a newly constructed house. Employee #1 was working in a trench that was approximately 11-ft deep by 4-ft wide which ran the entire length of the foundation. Employee #1 was killed when the trench collapsed. There was no method of protection from a cave-in being used. There was insufficient benching on the section of the trench that did not collapse. The soil type was tested and found to be Type "B." However, it appeared that the soil type was actually Type "C." The worksite conditions were dry with temperatures in the range of 48 degrees Fahrenheit. The work at the time of the accident was normal and did not deviate from the type of work usually performed. The owner received safety and health training and was knowledgeable of the excavation and trenching standards. Employee #1 was an American and spoke English.
