Investigation Summary
On March 15, 2005, Employee #1 was riding in a bucket attachment that was installed on the front-end of a John Deere agricultural tractor. He fell backwards from the bucket and was run over by the front tractor tire, killing him. The cause of death was determined to be due to massive internal injuries.
