Employee Killed When Machine Falls on Him

Employee #1 and the company owner were moving a Tajima Sewing Machine, Model Number TME-HC 612, Manufacture Number 12182-1, out of the building. Two 6-in.-diameter wheels had been placed under one end of the machine and the other end was raised on 2 in. by 4 in. blocks and a car jack. The machine fell over, crushed, and killed Employee #1. The sewing machine measured 199-in. long and was 38-in. wide at the table.
