Employee Is Killed When He Looses His Balance and Falls

On March 18, 2005, Employee #1 was installing metal roof decking on the roof of a church. Two coworkers were working from a staging area above lowering the decking sheets down to Employee #1 with ropes. They allowed the sheets to slide down the beams. Employee #1 removed the ropes from a sheet of decking and let them go. One rope became tangled around a support bracket and a coworker at the staging area, tried but could not free it. Employee #1 then unhooked his lanyard and proceeded out onto the I-beam to untangle the rope. He lost his balance and fell approximately 60 ft to the ground and died.
