Investigation Summary
At approximately 4:50 p.m. on February 11, 2005, an employee was allowed by the forklift operator to ride a large "Taylor" forklift that is rated at 62,000 lbs because the road was very muddy. The employee was riding standing on the right access platform as the forklift was backing into position to drop a load. The operator of the forklift stopped the forklift and turned the rear wheels in preparation for a left turn and gave the employee a hand signal so he could get off the forklift. Apparently, the employee was either standing in a blind spot or slipped and fell. The operator of the forklift did not see the employee and started moving forward to make the left turn and in the process the forklift ran over the employee with the rear tire. The employee was killed.
