Employee's Hand Is Crushed While Replacing Air Bag

On February 11, 2005, an employee, a machine operator, was working with a coworker in replacing an air bag in flat bed press, which was equipped with 45 air bags. The coworker did not lock out the pneumatic and electrical sources of energy. The air bags were located in the point of operation or danger zone of flat bed press. The coworker did not follow lockout-tagout procedures. Instead, he used vise grips to clamp air hose. While replacing air bag, he asked the employee to hold air bag in place, when suddenly the air bags filled with air, crushing the employee's hand.
