Investigation Summary
Employee #1 was securing a load of concrete panels onto a flatbed trailer. Employee #1 was working from a portable aluminum extension ladder at the front of the load while another employee was working at the back of the load. The employees were securing chains on the load. Employee #1 fell approximately 12-ft to 13-ft from the ladder to the ground when the side rails of the ladder broke. The ladder was a 16-ft light duty, 200-pound load, aluminum extension ladder. Employee #1 suffered head trauma, was transported to a hospital, and later died.
