Investigation Summary
On November 16, 2006, Employee #1 entered a 3,000 gallon chemical tank to retrieve an object, without the employer's permission. He exited the tank and then reentered it to dislodge the chain escape-type ladder which had become stuck at the bottom. While exiting the second time, a ladder rung broke and Employee #1 fell approximately 10 ft to the bottom of the tank. He became overcome by organic vapors. Employee #1 was rescued by local fire department personnel. He had cuts and bruises, and was not hospitalized.
