Employee Falls from Suspended Scaffolding

On June 1, Employee #1 and a coworker were on a two point suspended scaffold when one side detached from the rope pulley system. Employee #1 fell four stories to his death. The coworker was able to climb to the roof to safety. Neither Employee#1, nor the coworker was wearing harnesses and independent lifelines.
