Investigation Summary

A work crew was paving the shoulder of a two-lane, one-way highway. The work crew had erected a message board at appropriate intervals, traffic cones were put in place for approximately 2.5 miles, and a lateral buffer space of 12 ft was put in place. A traffic control plan was in place and a state inspector was on site at the time of the accident. Employee #1 and Employee #2 were working at ground level at the left side of the repaving box when they were struck by a pickup truck that veered and swerved past the safety cones and into the repaving vehicles that were located on the shoulder of the highway. Employee #1 and Employee #2 suffered concussions and were killed and Employee #3 suffered a broken arm and was hospitalized. The original form did not have an Injury Line for Employee #3.
