Employee Falls from Scaffold Work Platform

On February 14, 2005, Employee #1 and a coworker were disassembling an 18-ft high scaffold. They were working under a fall protection program that was set up by their employer. Employee #1 had relocated his tie off to the hand rail bracket on the scaffold platform. Employee #1 slipped or tripped, lost his balance and started to fall. His retractable life line was attached to his body harness and the other end to the hand rail bracket. The retractable life line worked, only allowing him to fall 6 ft, but the force placed on the hand rail bracket due to the sudden stop of the fall, caused the hand rail bracket and the platform that was attached to the hand rail, to also fall. The work platform and the hand rail bracket still attached to the platform landed on the ground next to Employee #1. Employee #1 received lacerations to his head.
