Investigation Summary

At approximately 1:10 p.m. on February 8, 2005, Employee #1, a contractor, was meeting with a subcontractor on a roof during a refabrication project. In the vicinity was an unguarded skylight, which was soon to be replaced. After their conversation, Employee #1 turned to walk across the roof and stepped through the skylight, falling to a concrete floor. He sustained a severe head injury. Employee #1 was transported to a hospital, where he died from his injuries.
