Investigation Summary
At approximately 9:15 p.m. on August 12, 2006, Employee # 1 was conducting maintenance on a heat exchanger. The process involved using a 90-ton crane to lift a piece of pipe known as a spool, approximately 60 ft onto a platform, and subsequently attaching it to the exchanger. The exact cause of the accident is unknown; however, witnesses stated that the crane operator temporarily laid the spool on the top of the exchanger to reposition the sling on one side. This was done because of a steel member that interfered with the lift. The spool shifted and struck Employee # 1. Emergency personnel were called, however, Employee # 1, later died at the site as a result of the injuries he sustained.
