Investigation Summary
On August 22, 2005, Employee #1 and the owner of the company were moving a 7.5-ft-high by 42-in.-deep by 36-in.-wide press from one area of the plant to another. The company owner used the Clark forklift and picked up the press by lifting the two, 4-in.-square hollow cross members, and traveled with the press approximately six inches off the floor. Employee #1 walked in front of the forklift to guide it. According to the owner, after he had the press in the area where he wanted it, Employee #1 grabbed the press pulling it off the forks. The press tilted and fell onto Employee #1 who, according to the owner, tried to catch it as it fell. The press landed across the Employee #1's chest, killing him.
