Employee Is Killed in Fall from Platform

At approximately 6:10 p.m. on August 4, 2005, an employee, a welder and pipefitter, climbed over the handrails of the permanent rig stairs after exiting the drill deck to access the personnel platform of an articulating boom lift, Snorkellift TB-A80. The employee slipped while climbing over the handrails and the personnel platform moved away from the stair landing as the employee placed his weight onto the platform rails, causing him to lose his balance and fall approximately 64 ft to the ground, killing him. The employee was wearing a fall protection harness with a Y-lanyard, both attachment ends of the Y-lanyard were attached to the back D-ring of the harness when the fall occurred.
