Investigation Summary
At approximately 8:15 a.m. on September 29, 2005, Employee # 1 and a coworker were installing a wood roof deck on a building. Employee # 1 untied a rope from the harness he was wearing to place a 4 ft by 8 ft sheet of plywood deck in an area he could not reach while he was tied off. Employee # 1 slipped and fell 37.2 ft to the dirt ground below. Employee # 1 was hospitalized and later died.
