Investigation Summary
Employee #1 was installing fascia on a 3-story residential home. While working from the third-floor deck, Employee #1 and two coworkers installed an outrigger scaffold assembly using 2-inch by 12-inch boards. The scaffold was approximately 5.5 feet from the deck level. Employee #1 requested a plastic bucket from one of the coworkers. After passing the bucket to Employee #1, the coworker heard and saw the nail gun hose moving quickly and with force. The coworker turned in the direction of Employee #1, did not see him, and then looked down to see Employee #1 lying on the ground. As Employee #1 attempted to climb onto the scaffold using a 5-gallon plastic bucket, he apparently reached out for the scaffold and fell. Employee #1 was transported by helicopter to Lee Memorial Hospital where he later died. There was no fall protection on the working deck or the scaffold assembly.
