Investigation Summary
On June 9, 2006, Employee #1 was cutting a section out of a water main in order to have it rerouted. He made the first cut on the south end successfully. When he tried to make the cut on the north side, the saw blade became hung up and immediately kicked back up into Employee #1's face, splitting his skin from the base of his neck to the tip of his nose. He required between 150 and 200 stitches. There were no broken bones. Neither the foreman, who was the designated competent person at the jobsite, nor Employee #1 knew that the diamond cutting blade on the chop saw was not appropriate for cutting PVC. The foreman had never inspected the blade or inquired if the diamond blade was meant to cut PVC. The owner and superintendent also did not know that the diamond blade was not approved for cutting PVC piping.
