Three Employees Sustain Fractures When Crane Tips Over

At approximately 8:50 a.m. on June 5, 2006, Employees #1-#6 were erecting a grain bin (105 feet in diameter; 80 feet high to the eaves; 109 feet high to the peak), using a Grove TMS 475 crane. The 105-foot diameter bin had its first three rings assembled, and the crew was just starting to install the roof. As the crane lifted and attempted to move the first section of roof truss into place, the crane started to tip forward. The crane operator attempted to pull the radius of the boom back toward him but did not succeed. The load pulled the crane over, and the boom came down on the grain bin. Employees #1-#4, who were working for Blackhawk, Inc., were standing on a sloped scaffold inside the bin waiting for the roof truss to be lowered so they could bolt it to the structure. The boom knocked the scaffold down, and Employees #1-#4 fell various distances (up to 35 feet) to the concrete surface below. Employee #6, who was working for Michel Concrete, was standing on the ground and was struck by the roof truss, when it came down. Employees #1-#6 were taken to the hospital. Employees #1-#3 were admitted to the hospital. Employees #4-#6 were treated and released from the hospital.
