Investigation Summary
On November 2, 2005, Employees #1, #2, and #3, working for Jeffrey Holliday Custom Framing, Inc., were installing windows at approximately 18 feet above the compacted ground. They were working from a job-built scaffold constructed of 2-inch by 4inch upright boards with 16-inch by 24-foot long "Engineered Floor Joists," when the scaffold collapsed. All three employees fell approximately 18 feet to the ground along with the scaffold. Employees #1 and #2 suffered fractures and were hospitalized. Employee #3 suffered contusions but did not require hospitalization.
