Investigation Summary
On March 25, 2009, an employee was trying to disconnect the brake lines to the trailer. He was then struck by, run-over and crushed by the right, passenger side drive wheels of a tractor truck. The employee was killed. The employee was not required, expected or in anyway authorized to be disconnecting the lines, and was doing so without knowledge of the driver of the truck.
