Investigation Summary
Employees #1 though #11, of Suntec Concrete Constructors, Inc., were pouring the concrete into wood forms using a concrete pumping truck. According to employee interviews, approximately 340 cubic yards had been poured when the deck collapsed and all 11 employees fell approximately 30 feet. Employees #1 through #11 were transported to the hospital for treatment of a wide range of injuries including broken arms, serious lacerations, and contusions. Employees #1, #6, and #8 were hospitalized. The area that collapsed was the southeast corner or Pour 17 which is the 4th floor deck. According to the design, under Pour 17, a third floor was not installed. Shoring towers supporting the deck spanned approximately 25 feet to the bottom of the forms. The entire deck collapsed including the plywood forms, rebar mat, and tensioning cables. The forms being filled were supported by Atlas All-speed shoring and Dako single post shores placed along with wood post shores. The specific beams that collapsed are identified on the architectural plans as beams PG-9, PB-4E, and PB-4C.
