Employee Is Injured When Struck by Forklift

On January 13, 2005, Employee #1 was operating a machine producing service parts. This operation is normally conducted on another shift. She bent over to count the number of parts in a basket. In the bent position, her buttocks were sticking out into an aisle. Two forklifts were traveling in the aisle in the opposite directions. The operator of one forklift moved over to allow enough room for the second forklift to pass. The operator of the second forklift was not aware that the machine was in operation and did not see Employee #1 bent over into the aisle. His forklift struck her and knocked her to the floor. When he heard someone screaming, he set a parts basket (weighing approximately 289 pounds) down and dismounted the forklift to see who was screaming. He inadvertently he set an edge of the parts basket down onto Employee #1's foot. She sustained contusions, abrasions, and a fractured toe.
