Employee Is Injured When Caught Up in Head Spool Chain

On June 24 2005, an employee was greasing a zert fitting on a belt separator head spool. As he attempted to complete his work his shirt or jacket got caught in the chain, sprocket and head spool shaft that was in close proximity to the work location. He was hospitalized and treated for arm amputation.
