Investigation Summary
Two employees were crawling across engineered joists while applying adhesive in preparation for the installation of the decking. The joists were approximately 21 ft, 2 in. long, spanning the entire width of the garage. On the east side of the garage, the joists were supported by a bearing plate and secured with a band board. On the east side of the garage, an approximately 10 ft, 6 in. section did not have the joists resting on a bearing plate. Due to a construction error, a portion of the foundation wall between the house and garage was not erected in this area so a bearing wall could not be constructed by the framers. According to Employee #1, a band board was attached to studs to which the joists were attached with two nails temporarily until the garage floor could be poured and a bearing wall constructed. Four joists broke free, turning on their sides, with one joists breaking, causing the both employees to fall approximately 11 ft, 10 in. to the compacted soil of the garage floor. One employee was not injured. However, Employee #1 struck the lateral bracing for the garage walls on the way down. Employee #1 sustained numerous contusions and muscle strains in addition to a fractured left orbital bone. Employee #1 was hospitalized.
