Employee's Fingertip Is Amputated by Unguarded Rip Saw

On February 4, 2005, Employee #1, of Prime Manufacturing, Inc., was rip cutting 4-ft by 4-ft pieces of plywood into 4.5-in. strips on a table saw. He was feeding the material into the saw by hand. He reached around behind the blade to help pull the material through the cut, and as he did, his left index fingertip came in contact with the blade. His fingertip was amputated back to the first knuckle. He was not hospitalized. The saw was not equipped with a blade guard, spreader, or anti-kickback device. The facility manufactures wood steps, staircases, and stair rails.
