Investigation Summary
On November 19, 2004, Employees #1 and #2, of a scaffold erection subcontractor, were erecting an addition to a fabricated frame scaffold that measured 72 ft by 44 ft. The addition measured 24 ft by 44 ft. During the erection, they were working 20 ft 6 in. above the concrete floor below. The employees were not utilizing any means of fall protection, while they erected the fabricated frame scaffolding. The employer did not have a fall protection plan for the work being conducted. Employee #1 was placing 2 in. by 12 in. by 8 ft and 12 ft planks every other board leaving approximately 12 in. between each board. Employee #1 would then walk across the boards already placed to retrieve another board and then walk back across and place the new board. Employee #2 was placing the plywood decking. Employee #1 placing the planking stepped onto a board already in place and that board snapped. Employee #1 fell to the ground below with Employee #2 of another contractor breaking his fall and his leg in the process. Employee #2 that fell received cuts to his arm and refused treatment at the area hospital and was only treated at the scene. This employer was not utilizing any means of fall protection nor was they utilizing a safety monitor as both employees on site were working independently of each other. Neither employee was hospitalized.
