Two Employees Are Killed in Fall from Scaffold

On March 1, 2006, Employees #1 and #2, and three coworkers, were working from a scaffold on a building that was under construction. The scaffold, an Outinord Universal stripping platform, was part of a tunnel form system that extended to the twentieth story of the building. As Employees #1 and #2, and their coworkers, were walking to an elevator at the end of the platform to go on a break, the platform collapsed. The three coworkers grabbed onto the nearby guardrail and net system and were later rescued. However, Employees #1 and #2 fell twenty stories to the ground and were killed. The accident investigation revealed that the structural design of the platform was flawed, due to the frame not being appropriately supported; the outer brace was not correctly proportioned; and the weight of the employees, in one area, was not properly calculated.
