Employee Dies after Being Caught in Forklift

At approximately 2:00 p.m. on April 11, 2005, Employee #1 became trapped between the mast and the cab of a forklift while he was accessing the driver's seat through the front of the cab. Employee #1 died later that day from the injuries he received.
