Employee Is Crushed and Killed by Falling Load of Steel

At approximately 8:00 a.m. on April 13, 2005, Employee #1 was using a forklift to pick up 7,500 pounds of steel and was walking up to the forklift to attach and chain through an in-house-made device which was sitting on the two front forks of the forklift. Employee #1 was crushed when the 7,500 pounds of steel fell over on him. Employee #1 was killed.
