Employee's finger is amputated in patty machine

On September 25, 2004, an employee, General Manager of the restaurant, was operating the Hollymatic Patty Machine Super Model Number 54. The regular patty machine operator had not shown up for his shift, and the employee was responsible for producing patties for the day. He was the only employee in the Butcher shop. The paper sheet dispensing system was not in use at the time of the incident. The employee was wearing latex gloves and was holding his left hand directly below mold plate and catching the patties immediately as they punched out of the machine. As the empty mold plate retracted, the end of the glove on his left middle finger was caught and the tip of the finger was amputated above the joint. All machine guards were in place and functioning properly at the time of the incident. The employee was not hospitalized.
