Investigation Summary
On November 16, 2004, Employee #1 and a coworker were installing an LVL beam overhead. The beam measured 18-in. by 1.75-in. by 25-ft by 7.5-in., and weighed over 200 pounds. The beams were to be three deep that would rest in a steel bracket measuring approximately 12-in. tall by 5.5-in. wide that was installed from a step ladder that was previously used, but was used in the open position. The beam to be installed was resting in an almost level position; the left side was on the fourth or fifth step of the 10-foot ladder, and right side of the beam resting on an intermediate portion of the north wall. Employee #1 picked up the beam that was resting on the step ladder, and placed it on his right shoulder and held it with his right hand and began to climb the 10-foot step ladder until he reached the fourth or fifth step putting him approximately 4 ft above the floor, and in the range of the bracket 9 ft above the floor. The ladder was in the closed position during this process. Employee #1 was attempting to place the beam in the bracket while his coworker was holding the other end, helping to keep the beam steady and guide it into the bracket, when the load shifted. Employee #1 tried to lean toward the wall on his right to prevent himself from falling, but the wall was not within reaching distance. Employee #1 fell from the ladder with the beam, landed on his feet and the beam struck Employee #1 in his back and shoulder which knocked the air out of him and forced him to the ground. A coworker summoned emergency medical services and Employee # 1 was transported to the local fire department, where he was transported to the regional medical center for treatment and released approximately 7 hours later.
