Employee Is Killed in Fall from Roof

Employee #1 was replacing a cooling tower on the roof of a six-story building. The area Employee #1 was working in at the time of the incident was within 4 ft of the edge of the roof. Along the edge was a parapet wall 17 in. high. No fall protection was being used. Employee #1 was killed.
