Employee Is Killed in Fall from Ladder

On December 16, 2004, Employee #1, the sole proprietor of Holloway Metal Constructors, placed a metal extension ladder against a wall girder to access the girder and secure the ladder to it. While climbing, Employee #1 fell approximately 6 ft to the ground. He struck his head and was killed.
