Employee's Fingers Are Crushed in Depalleter

On October 11, 2004, Employee #1, a machine operator, un-jammed a block depalleter machine and was watching it to ensure that it did not jam again. He placed his left hand on top of the machine and the return stroke of the pusher arm pinched his middle and ring fingers. His fingers were severely crushed and he lost the tip of his ring finger and was hospitalized.
