Employee's Fingers Are Amputated by Unguarded Table Saw

Employee #1 was operating a sliding table saw to cut 0.125 in. strips from a piece of oak lumber. Employee #1 moved the hood guard out of the way and started to cut. The spreader and anti-kickback dogs were not installed. The board kicked up on the moving blade. Employee #1 reached with his left hand to push the board down. The blade amputated his left index and middle fingers.
