Employee's Finger Amputated by Machine Clamp

On November 11, 2005, Employee #1 was operating a Castle screw pocket machine (model TSM 20), when a descending material clamp caught his right middle finger, which was amputated as a result. The space between the clamp guard and the work table was 1.5 in. and the material being processed was 0.75 in., allowing a 0.75-in. space for Employee's #1 finger to enter the point of operation.
