Employee Amputates Finger in Crushing Incident

At approximately 12:05 a.m. on March 15, 2005, Employee #1 and a coworker were using a forklift to support the front axle of a grader in order to perform repairs, which included the replacement of a part. In addition to supporting the axle of the grader, the forklift was used to transport and support the replacement part. The replacement part had been angled across the forks to help with installation, but was properly aligned, requiring to be moved 3 to 4 inches. As the coworker walked towards the replacement part, near the back of the forklift, to move the part, Employee #1 tried to manually move the part. The part shifted and came into contact with Employee #1's left middle finger tip, which was crushed between the replacement part and the frame of the grader. The tip of Employee #1's left middle finger was amputated.
