Employee is injured by fall from work platform on forklift

On September 17, 2004, Employee #1, a laborer, was in a fruit bin on the lifting carriage of a forklift, changing overhead light bulbs at the building entrance. After changing the light bulb, the forklift operator was lowering Employee #1 to the ground when the worker moved to one side of the platform, causing the platform and Employee #1 to fall from the forks, approximately 5 feet to the concrete floor below. Employee #1 was hospitalized with a fractured hip.
